Estrogen therapy is commonly used for transgender women and AMAB nonbinary people. Estrogen is usually taken in pill form, but can also be applied as a patch, gel, or injection. Estrogen promotes breast growth, softens skin, decreases erectile function, and more. An anti-androgen is often prescribed alongside estrogen to block testosterone. Common estrogens used in HRT include estradiol, estropipate, and conjugated estrogens.

Testosterone therapy is commonly used for transgender men and AFAB nonbinary people. Testosterone is most often given via intramuscular injection, but can also be applied as a gel or patch. Testosterone causes facial and body hair growth, voice deepening, muscle growth, fat redistribution, and more. Testosterone types include testosterone cypionate, testosterone enanthate, and testosterone undecanoate.

Progesterone may be added to an estrogen regimen for fuller breast development for transfeminine patients. However, its necessity is debated. Progesterone is usually taken orally in pill form.

GnRH agonists (puberty blockers) can be used to suppress estrogen or testosterone production. This delays pubertal changes in adolescents and reduces hormone levels in adults. Brand names include Lupron, Supprelin LA and Triptodur.

5-alpha reductase inhibitors like finasteride block the conversion of testosterone into DHT, minimizing hair loss and other masculinizing effects.

Beyond medications, hormone levels can also be modified via surgery. Orchiectomy removes the testicles to decrease testosterone production in AMAB patients. Oophorectomy removes the ovaries to decrease estrogen production in AFAB patients.
